ABSTRACT
BACKGROUND: Sodium-glucose cotransporter 2 inhibitors (SGLT2i) have proved beneficial effects on cardiovascular risk and preservation of renal function in patients with diabetes mellitus and chronic kidney disease, in addition to their glucose-lowering effects. This study aims to investigate whether SGLT2i was associated with a more favorable metabolic profile in kidney transplant recipients with post-transplantation diabetes mellitus (PTDM).
METHODS: In this observational study, patients receiving a kidney transplant between 2007 and 2021 in Norway were identified from the national transplant database. Patients who developed PTDM were identified through new- onset prescription of glucose- lowering treatment after kidney transplantation registered in Norwegian Prescription Database. PTDM patients were categorized whether glucose- lowering treatment included SGLT2i or not during the first 4 years after transplantation. Metabolic parameters at 5 years after transplantation reported to the Norwegian Renal Registry was compared in the 2 patient groups.
RESULTS: This observational study included 232 kidney transplant recipients with PTDM. Metabolic parameters at 5 years post-transplantation were compared among the SGLT2i-group (n = 39) and the control group (n = 193); median BMI (27.5 vs. 26.5 kg/m2, p = .22), diastolic blood pressure (80 vs. 79 mm Hg, p = .40), systolic blood pressure (129 vs. 132 mm Hg, p = .42), proteinuria-parameters (u-ACR 4.5 vs. 3.5 mg/mmol, p = .33), (u-PCR 26 vs. 13 mg/mmol, p = .29), HbA1c (52 vs. 49 mmol/mol, p = .11) and estimated glomerular filtration rate (eGFR, 61 vs. 56 mL/min/1.73m2, p = .20).
CONCLUSION: Our study could not demonstrate improved metabolic parameters in PTDM patients treated with SGLT2-inhibitors at 5 years post transplantation.
