ABSTRACT

Hypertrophic cardiomyopathy (HCM) is a myocardial disease characterized by left ventricular hypertrophy without abnormal loading conditions. Despite significant advances in management, HCM remains a major cause of sudden cardiac death, particularly in young individuals. Prevention relies on accurate arrhythmic risk stratification and the selective implantation of implantable cardioverter-defibrillators (ICDs). This article reviews current indications for primary prevention ICD implantation in sarcomeric HCM based on the European Society of Cardiology (ESC) guidelines and compares them with the more integrative approach proposed by the American College of Cardiology/American Heart Association (ACC/AHA).
