The impact of obesity on postoperative pulmonary complications in patients undergoing aortic surgery

ABSTRACT

BACKGROUND: Postoperative pulmonary complications (PPCs) are common after surgery in patients with acute type A aortic dissection (ATAAD). This study aimed to evaluate the incidence and risk factors for PPCs in ATAAD patients.

METHODS: This single-center retrospective study included 203 patients with ATAAD who underwent urgent aortic surgery between January 2023 and August 2024 Patients were divided into PPCs and non-PPCs groups. The incidence and types of PPCs were recorded, and risk factors were analyzed using multivariable logistic regression. Postoperative outcomes were compared between two groups.

RESULTS: Patients undergoing surgery for ATAAD, 36 (17.7%) developed PPCs. Multivariable logistic regression analysis identified obese as an independent risk factor for PPCs (adjusted OR 3.805, 95% CI 1.408-10.285, P = 0.008).

CONCLUSIONS: Obese was an independent risk factor for PPCs in patients undergoing surgery for ATAAD. These findings highlight the importance of preoperative BMI assessment in optimizing perioperative management.
