ABSTRACT

Chronic obstructive pulmonary disease is a progressive, multisystem condition associated with substantial morbidity, mortality and healthcare burden. In the UK, and worldwide, it remains a leading cause of death, with smoking and environmental exposures as primary risk factors. Community nurses play a pivotal role in coordinating integrated care, supporting self‑management, addressing polypharmacy and acting as a key conduit between patients, families and multidisciplinary teams.
