ABSTRACT

Comprehensive cardiovascular rehabilitation and prevention programs (CVRhP-P) are the cornerstone for improving maximum exercise tolerance, reducing mortality, improving health-related quality of life, and reducing healthcare costs in patients with cardiovascular disease (CVD). These types of programs have grown relatively slowly since their inception in the first decade of the last century, although the last 10 years have seen an increase in the number of centers, both in developing and high-income countries. In order to update and standardize scientific knowledge in this clinical area, the SOMECCOR and SMC task force developed these clinical practice guidelines based on real-world problems (PICO) and a strict, internationally approved methodology, based on AGREE-II and the Delphi method. The thematic spectrum begins with care for hospitalized and critically ill patients and continues through to long-term preventive measures, always with a focus on therapeutic physical training and a heart-healthy lifestyle. These CPGs are intended for healthcare personnel in various disciplines, as well as administrative, managerial, and government professionals. The recommendations also include a cost-benefit analysis and tips for implementation. Some topics of interest include the type of unit appropriate for patient risk, prehabilitation, telemedicine, advice on daily living for patients with heart disease (driving, air travel, etc.), dysautonomia, emergency care protocols, complexity sciences, training planning, and process management, among others. The SOMECCOR/SMC Guidelines also do not relieve healthcare professionals from the obligation to consider relevant updated official recommendations or guidelines fully and carefully from the competent health authorities to manage each patient's case in the light of scientifically accepted data, in accordance with their respective ethical and professional obligations. In addition, it is the responsibility of the healthcare professional to check the applicable standards and regulations for medicinal products and medical devices at the time of prescription. SOMECCOR/SMC warns readers that technical language may be misinterpreted and declines any responsibility in this regard.
