ABSTRACT
INTRODUCTION: Colchicine has been used for centuries in the management of gout and familial Mediterranean fever. Its clinical applications now extend to pericarditis, systemic inflammatory conditions, and the prevention of cardiovascular events. Despite its benefits, colchicine's narrow therapeutic window and complex metabolism place patients at a high risk of severe toxicity, particularly in those with renal or hepatic impairment, advanced age, or polypharmacy.
METHODS: This review aimed to synthesize current evidence regarding colchicine pharmacology, mechanisms of toxicity, risk factors, clinical presentation, diagnostic considerations, and management strategies. Emphasis is placed on implications for nursing practice and the critical role of timely recognition and supportive care.
RESULTS: Colchicine toxicity most commonly presents initially with gastrointestinal symptoms, progressing to multiorgan dysfunction including bone marrow suppression, hepatic and renal failure, and cardiovascular collapse. Chronic exposure may result in neuromyopathy and cytopenia. No antidote exists; management relies on supportive measures such as early gastrointestinal decontamination, fluid and electrolyte replacement, vasopressor support, and, in severe cases, extracorporeal membrane oxygenation. Adjunctive therapies, including intravenous lipid emulsion and N-acetylcysteine, have shown promise in reducing cellular damage. Patient populations at the highest risk include older adults, individuals with organ dysfunction, pediatric patients with accidental ingestion, and those receiving interacting drugs such as macrolides, azole antifungals, or certain statins.
DISCUSSION: Colchicine remains a valuable therapeutic agent, but its safety demands vigilance. Emergency nurses and providers play a pivotal role in early detection, risk reduction, and patient education. Proactive monitoring and adherence to dosing guidelines are essential to maximize therapeutic benefits while minimizing toxicity and mortality.
