ABSTRACT
BACKGROUND: This project at Conway Medical Center focuses on patients as they transition from acute care to ongoing recovery. By addressing gaps in communication, patient education, and care coordination through targeted interventions and dedicated staffing roles, we aim to ensure patients are prepared for a successful recovery.
PROJECT RATIONALE: This project stems from the recognition that a successful discharge is essential to provide safe, consistent, and patient-centered care. Conway Medical Center optimized the discharge process for ST-elevation myocardial infarction and non-ST-elevation myocardial infarction patients to reduce readmissions and facilitate seamless care transitions.
PROJECT SUMMARY: Our patient-centered approach has led to significant improvements in discharge performance. By focusing on personalized education, safety, and support, we strive to empower patients throughout their recovery journey and enhance their overall experience.
TAKE-HOME MESSAGE: Discharge should be the beginning of the journey for ongoing care and recovery, not the conclusion.
